Simply fax your request in letter form with the following information on SFUSD School or SFUSD Department letterhead:
- Date and notation that request is for an Affinity Grant
- Your Name, Position at SFUSD and relationship to Student
- School Name or SFUSD Dept., Address, Tel and Fax Numbers
- Name & description of student(s) on whose behalf you are submitting request, school student(s) attends, student’s “Affinity”, and reason for grant – (a compelling description of the circumstances that led you to submit an application for this specific student or group of students.)
- What services are you proposing for this student(s)? Describe proposed activity. If available, attach printed materials on activity and instructor. Provide start and end dates of activity; how many students involved, ages of students, etc.
- Indicate the details you have worked out regarding the place for the activity, transportation and the plan to ensure student(s) will attend regularly. Can student(s) receive service at the after-school program?
- Where/how will the student(s) enroll – name and contact information of organization and/or resource person/instructor.
- Evidence of “low income”, e.g. qualifies for free and reduced lunch,
- Indicate whether you have parent/guardian permission.
- Describe funding from other sources that you are applying for
- Total amount requested with itemization of how money will be spent (per session cost, flat fee, supplies, etc.).
- Grant requests must be signed by the submitting SFUSD professional AND the School Principal or SFUSD Dept Supervisor
Funding is limited. Submit your request as soon as it is ready. Responses to requests will be reviewed upon receipt.
Grant applicants will be expected to submit all receipts and evidence of participation. Pictures and letters are encouraged.
